Knowsley Looked after Children Health Team
A Summary
The Knowsley looked after children health team is a small team based in Knowsley and employed by a local National Health Service (NHS) trust. The team has two nurses, Jackie Riley, Named Nurse for looked after children and Rebecca (Bex) Holt, Specialist Nurse for looked after children. We also have admin support from Chris Moore and will be recruiting another part time team member to help support Chris in his admin role.
The team has responsibility to:
- Make sure that your health needs are met by all of the health services in the community and sometimes in hospital settings no matter where you live.
- We make sure that children have a new health check when they come in to care and also a regular times to make sure everything is okay (with a specialist children’s Doctor, (paediatrician).
Care leavers are provided with a Health Passport by the team. This is completed by one of the nurses and is supported by the information shared by health services and those supporting you. The health passport provides health information we have on our electronic records system and includes details of when you were born, what time and your weight at birth amongst other things.
The team is also available to provide you with advice, signposting to services and support with health issues or worries you might have about your health.
